Can anyone recommend a journal that is formatted to be a reading journal? I don't want to use a blank book. I want it to have headers for author, title, notes, etc.

Try the Book Lust series of books by Nancy Pearl. Along with books about reading they have a journal that's like a Books I've Read organizer.
